Accutane (isotretinoin) has a mixed reputation, but among dermatologists it’s the finisher for patients with severe acne. “If you have an acne patient that doesn’t respond to anything, [Accutane] can really be a game changer,” board-certified dermatologist Adam Friedman tells SELF. Accutane is an oral retinoid, and it has all the same benefits of a topical retinol but is even more effective.

Acne, also known as acne vulgaris, is a long-term skin disease that occurs when hair follicles are clogged with dead skin cells and oil from the skin.[10] It is characterized by blackheads or whiteheads, pimples, oily skin, and possible scarring.[1][2][11] It primarily affects areas of the skin with a relatively high number of oil glands, including the face, upper part of the chest, and back.[12] The resulting appearance can lead to anxiety, reduced self-esteem and, in extreme cases, depression or thoughts of suicide.[3][4]
The proper care for problem skin should begin with a competent cleansing. Do not wash more often than 2 times a day. Too frequent washing contributes to the regular removal of sebum, which leads to its further increased secretion and provokes acne even more. The temperature of the water for washing should not be too hot, it expands the already wide pores, which only increases the production of sebum and raises the risk of acne.

This problem is normally located on the face, back, and upper portion of the chest. These areas are worst hit because they have the highest number of oily glands. What a lot of people don’t realize is that there are several different types of this issue. And to make matter even more complicated, each of these types have their own causes and effects as well as methods of prevention. [3]
Tretinoin: As we said above, tretinoin (common brand name: Retin-A) is a synthetic retinoid, but it is stronger than some of the other options, and its cousin, isotretinoin, is even stronger. Isotretinoin, better known as Accutane, is an oral synthetic retinoid typically only prescribed for very severe cases of cystic acne because it can cause intense side effects and is a powerful teratogen, meaning it causes birth defects. However, after taking isotretinoin for several months, many people never need to do any serious acne treatment again, so for some, it is well worth the side effects.
Although there is not enough research to determine if certain foods cause breakouts, there are certain changes you can make to your diet to help prevent them. For breakfast, switching to plain oatmeal that has only been sweetened with fresh fruit can help prevent excessive androgen production. When you eat fish, opt for salmon, which is high in omega-3s. One omega-3 called DHA is an anti-inflammatory. Snacking on sunflower seeds can give you more vitamin E in your diet. Vitamin E helps the immune system fight off bacteria before inflammation and cysts occur. Finally, make sure you stay hydrated by drinking plenty of water each day. Your body needs almost half a gallon each day.
This is a low foaming formula that doesn’t produce much lather or foam which, for some, may be quite drying. It’s non-comedogenic and moisturizing, without being oily, leaving skin feeling soft and smooth. As the name suggests, it’s the best normal and oily skin types, and for some with combination skin. Due to its low-foaming nature, it doesn’t strip skin of natural oils, promoting skin health and healthy cell growth and protection. However, it may not be good for people with sensitive skin types.
If you’re looking for a hormonal solution to your acne but don’t want to take a combined oral contraceptive, spironolactone may be the answer. This oral medication is a potassium-sparing diuretic originally designed to treat high blood pressure, but is now also used to treat acne. It mainly functions by reducing sebum production, leading to less acne formation13. If you are able to get pregnant, you don’t necessarily need to take a combined oral contraceptive, but you will want to use some form of birth control since spironolactone, like Accutane, is a well-known teratogen and is known for causing birth defects.
Acne is caused when the sebaceous glands in your skin get infected or inflamed. Blemishes and pimples are most commonly found on your face, chest and back. You have a system of oil glands under your skin, and this is where sebum is produced. Sebum is supposed to keep your skin healthily moisturized, but too much production of this oil can cause blackheads and whiteheads. When the oil is pushed out through your hair follicles, it can clog your pores if it attaches to bacteria or dead skin cells. If this happens, sebum will build up behind the plug and form a pimple. And we all know what that is.
The Body Shop Tea Tree Oil Products: Even though it’s not made in a lab, tea tree oil has been studied and proven to be a powerful anti-acne agent. The Body Shop takes advantage of this ingredient with their full line of tea tree oil products, from pure oil to face wipes to a clay mask, all containing tea tree oil. These products are best for oily skin, pimples, and cystic acne.
Having a specific type of skin now doesn’t mean you will be stuck with it the rest of your life. As you get older, your skin can change. It may be oily when you are younger but become dry as you age. Plus, skin can become more or less sensitive over time. It’s always a good idea to retest for your skin type if you notice any changes. We’ve included a guide above to help you recognize what kind of acne you have, what the average causes are, and how to treat it (or at least not aggravate it).
The active substance of the gel cleanses the top layer of skin from dead cells, normalizes the work of the sebaceous glands, evens the complexion, stimulates the natural production of collagen and elastin. For cleansing the skin, a small amount of gel is enough twice a day; you can also use the product as an addition to ordinary facial cleansers. The skin gets rid of excessive dryness, redness and pigment spots, acquires elasticity and freshness. The gel can be used for any type of skin, including oily and combination.

Conversely, not using any facewash on your skin leaves you with the build-up of dead skin cells, dirt, grime and excess oils. While many others swear by using the simple combination of warm water and a facial cloth for their skincare routine, warm water isn’t always enough to penetrate your oil-clogged pores that have dirt and bacteria trapped inside. If you think you might disagree, imagine rinsing an oily, used frying pan with just warm water. Without a proper cleanser, you can’t cut into the grease buildup or eliminate harmful bacteria. The same applies for your skin.
However, salicylic acid isn’t just for those of us with sensitive skin. It can also help those of you with tougher skin, through higher concentrations. Over-the-counter, you can find salicylic acid in concentrations up to 2 percent, but if you want something even more intense, many spas and dermatology offices offer salicylic acid chemical peels with 20-30 percent salicylic acid. Beware, these peels will likely leave your face very photosensitive for a few days, but they have been known to significantly reduce sebum for a few weeks at a time.
Clean your face really well using a mild face cleanser and gently pat dry. Let your skin further air dry and don’t put anything else on it. Don’t put anything else on your face like moisturizer, serum, toner, etc. After a half hour, carefully check your cheeks, chin, forehead and nose, with a hand mirror, for any shine. Also, check if your skin feels tight when you smile or make exaggerated facial expressions.
Retinoids and retinoid-like drugs. These come as creams, gels and lotions. Retinoid drugs are derived from vitamin A and include tretinoin (Avita, Retin-A, others), adapalene (Differin) and tazarotene (Tazorac, Avage). You apply this medication in the evening, beginning with three times a week, then daily as your skin becomes used to it. It works by preventing plugging of the hair follicles.

This ties back into what I was saying about your face being delicate. The way you clean your underarms, or upper torso, should most definitely not be the way you clean your face. The former body parts are washed by scrubbing, usually in a vigorous manner. Your face is far too delicate for this. Rough scrubbing on your face will only irritate it and cause problems.
Frequently used topical retinoids include adapalene, isotretinoin, retinol, tazarotene, and tretinoin.[46] They often cause an initial flare-up of acne and facial flushing, and can cause significant skin irritation. Generally speaking, retinoids increase the skin's sensitivity to sunlight and are therefore recommended for use at night.[1] Tretinoin is the least expensive of the topical retinoids and is the most irritating to the skin, whereas adapalene is the least irritating to the skin but costs significantly more.[1][84] Tazarotene is the most effective and expensive topical retinoid, but is not as well-tolerated.[1][84] Retinol is a form of vitamin A that has similar but milder effects, and is used in many over-the-counter moisturizers and other topical products.
@ brazen i also get exposed to sunlight a lot and would recommend Cetaphil as it has no photosensitive effects. I use Cetaphil wash and it has really been a great help with my acne. it cleans your skin thoroughly while still being gentle. i would recommend using this with a cleansing brush (clarisonic, luna, spin brush etc) and following with a toner preferably one with witch hazel. these combination of things has worked wonders for my skin. 3 months down the line the improvement has been fantastic. All I’m dealing with now is the scars (Hyperpigmentations).
Accutane is especially good for cystic acne in women and body acne in men. “Oral vitamin A basically shuts down your sebaceous glands. If you suppress [them] for a long enough period, you can cure someone of their acne, and about 50 percent do hit that cure rate,” says Linkner. A course of Accutane can take about six to nine months. Sometimes patients need to repeat the course at a higher dosage in order to truly eliminate acne.
