The earliest pathologic change is the formation of a plug (a microcomedone), which is driven primarily by excessive growth, reproduction, and accumulation of skin cells in the hair follicle.[1] In normal skin, the skin cells that have died come up to the surface and exit the pore of the hair follicle.[10] However, increased production of oily sebum in those with acne causes the dead skin cells to stick together.[10] The accumulation of dead skin cell debris and oily sebum blocks the pore of the hair follicle, thus forming the microcomedone.[10] This is further exacerbated by the biofilm created by P. acnes within the hair follicle.[44] If the microcomedone is superficial within the hair follicle, the skin pigment melanin is exposed to air, resulting in its oxidation and dark appearance (known as a blackhead or open comedo).[1][10][19] In contrast, if the microcomedone occurs deep within the hair follicle, this causes the formation of a whitehead (known as a closed comedo).[1][10]

Antibiotics for acne, both topical and oral, used to be the top prescriptions for getting clear skin, but research has revealed that they aren’t the best option for most people today due to something called antibacterial resistance. This is where bacteria mutate, become immune to certain antibiotics, then reproduce offspring that are also immune, creating entire colonies of bacteria that can’t be killed by certain antibiotics.
Although some people think they can get the best acne treatment by mixing and matching products, this is not the recommended method of treating blemishes. Aside from doctors, few people understand how different products can react with each other. In some cases, combining two products increases how well both work. For example, green tea extract and salicylic acid complement each other. Skin care system makers usually hire doctors to help improve the effectiveness of their systems.
According to Dr. Bailey, it’s not enough to have a facial cleanser designed for acne-prone skin. Half the battle is properly washing your face. Here’s what she recommends: “First, wet your face with warm water, then lather your cleanser over all of your facial skin. Depending on your skin type, you can use your fingers, an exfoliating cloth or a sonic skin-cleansing brush system. Rinse well with warm water to remove the lather entirely. Some of the important acne cleanser ingredients will stay behind, but the cleanser’s foaming agents, as well as built-up oil, dead cells, products and bacteria, will be rinsed off of your skin.”
The predisposition to acne for specific individuals is likely explained by a genetic component, a theory which is supported by studies examining the rates of acne among twins and first-degree relatives.[2] Severe acne may be associated with XYY syndrome.[39] Acne susceptibility is likely due to the influence of multiple genes, as the disease does not follow a classic (Mendelian) inheritance pattern. Multiple gene candidates have been proposed including certain variations in tumor necrosis factor-alpha (TNF-alpha), IL-1 alpha, and CYP1A1 genes, among others.[18] Increased risk is associated with the 308 G/A single nucleotide polymorphism variation in the gene for TNF.[40]

It doesn’t matter how much money you spend on it. It is important that the acne remedy works.

According to a 2016 review of research that examined how diet may impact breakouts, researchers concluded that “compelling evidence shows that high glycemic load diets may exacerbate acne.” Foods high on the glycemic index (GI) tend to be higher in refined carbs, like those found in white bread. Scientists suspect that raised insulin levels from the carbs may trigger a release of hormones that inflame follicles and increase oil production.

A major mechanism of acne-related skin inflammation is mediated by P. acnes's ability to bind and activate a class of immune system receptors known as toll-like receptors (TLRs), especially TLR2 and TLR4.[44][64][65] Activation of TLR2 and TLR4 by P. acnes leads to increased secretion of IL-1α, IL-8, and TNF-α.[44] Release of these inflammatory signals attracts various immune cells to the hair follicle including neutrophils, macrophages, and Th1 cells.[44] IL-1α stimulates increased skin cell activity and reproduction, which in turn fuels comedo development.[44] Furthermore, sebaceous gland cells produce more antimicrobial peptides, such as HBD1 and HBD2, in response to binding of TLR2 and TLR4.[44]
