What it is: Originally under the brand name "Accutane" but now available only in generic form, isotretinoin is a derivative of vitamin A taken in pill form for 15-20 weeks. Doctors normally prescribe it for people with "severe nodular acne" that does not respond to other treatments. Nodules are inflammatory lesions with a diameter of 5mm or more. That’s why, no matter how uncomfortable your skin may feel while plagued with acne, you must resist the urge to touch your skin. If the irritating sensations become unbearable, there are other methods of treating your skin, such as cooling it with ice packs or aloe vera gel. You can even use medicated creams designed to soothe irritated skin – given that your dermatologist says it’s okay.

Students with acne were examined before and after major exams at school, and their acne got worse when they experienced stress before exams. It is a double-edged sword. Acne can cause stress, but it can also get worse with stress. Stress hormones such as cortisol can overstimulate the oil glands in your skin. And we already know that oil, bacteria and dead skin cells are what really cause acne. So try to keep away from stressors while you try to get your skin healthier.

Acne vulgaris is a chronic skin disease of the pilosebaceous unit and develops due to blockages in the skin's hair follicles. These blockages are thought to occur as a result of the following four abnormal processes: a higher than normal amount of oily sebum production (influenced by androgens), excessive deposition of the protein keratin leading to comedo formation, colonization of the follicle by Propionibacterium acnes (P. acnes) bacteria, and the local release of pro-inflammatory chemicals in the skin.[49]
If a pore is open, it’s called a blackhead or “open comedo”. Because of the opening, contact with the air oxidizes the dead skin cells inside the pore. The air contact turns the melanin inside them darker in color, similar to the way a peeled banana left exposed to the air will darken. This is how the blackhead forms. The color of a blackhead is not because of dirt. It’s a mixture of air and the skin pigment called melanin.

Hormonal activity, such as occurs during menstrual cycles and puberty, may contribute to the formation of acne. Tretinoin: As we said above, tretinoin (common brand name: Retin-A) is a synthetic retinoid, but it is stronger than some of the other options, and its cousin, isotretinoin, is even stronger. Isotretinoin, better known as Accutane, is an oral synthetic retinoid typically only prescribed for very severe cases of cystic acne because it can cause intense side effects and is a powerful teratogen, meaning it causes birth defects. However, after taking isotretinoin for several months, many people never need to do any serious acne treatment again, so for some, it is well worth the side effects.
The skin care products you apply to your face regularly can have a big impact on your complexion. You shouldn’t skip moisturizing if you have acne, especially if you’re using drying treatments—but the type of moisturizer you use can make a difference. “Even acne-prone teenagers need to moisturize to keep their skin barrier healthy. The skin barrier can become damaged from drying acne products and medications,” Arielle Kauvar, MD, director of New York Laser & Skin Care and clinical professor of dermatology at New York University School of Medicine, recently told Prevention.
No one knows exactly what causes acne. Hormone changes, such as those during the teenage years and pregnancy, probably play a role. There are many myths about what causes acne. Chocolate and greasy foods are often blamed, but there is little evidence that foods have much effect on acne in most people. Another common myth is that dirty skin causes acne; however, blackheads and pimples are not caused by dirt. Stress doesn't cause acne, but stress can make it worse.
Oral contraceptives can help normalize hormonal surges and regulate monthly cycles so that oil glands don’t go into overdrive, says Dr. Zeichner. Doctors may prescribe one of four brands of birth control pills—Yaz, Beyaz, Estrostep, and Ortho Tri-Cyclen—that are FDA approved for treating acne. As always, patients taking oral contraceptives should be aware of potential birth control side effects, including blood clots or vaginal dryness.
Azelaic acid has been shown to be effective for mild to moderate acne when applied topically at a 20% concentration.[66][129] Treatment twice daily for six months is necessary, and is as effective as topical benzoyl peroxide 5%, isotretinoin 0.05%, and erythromycin 2%.[130] Azelaic acid is thought to be an effective acne treatment due to its ability to reduce skin cell accumulation in the follicle, and its antibacterial and anti-inflammatory properties.[66] It has a slight skin-lightening effect due to its ability to inhibit melanin synthesis, and is therefore useful in treating of individuals with acne who are also affected by postinflammatory hyperpigmentation.[1] Azelaic acid may cause skin irritation but is otherwise very safe.[131] It is less effective and more expensive than retinoids.[1]
